Sholom Home West is a long-term senior care agency that accommodates up to 175 residents. The home is licensed both for skilled nursing care and intermediate care. It provides additional services such as physical and occupational therapy, recreation, and medical care. In cooperation with Minneapolis Family and Children's Service and St. Paul Jewish Family Service, the home provides day care and kosher meals delivered to home-bound Twin Cities residents.

Sholom Home's Ackerberg Community Campus includes the Roitenberg Family Assisted Living Residence, opened in October 2002. The residence, at 3610 Phillips Parkway, offers 52 traditional assisted living units and 24 "Memory Care" units. For information, call (952) 708-1776.
